Appointment Information for Restoration Physical Therapy :
Call or email to set up an appointment. Initial appointments include an evaluation so allow 1 to 1-1/2 hrs. Your therapist will take a detailed history and utilize objective tests to assess current areas to address in your treatment session. Repeat visits are 45 min. to 1 hr. To expedite the intake process, please download and complete the Patient Information Form below. If you are scheduled for Bowenwork manual therapy, dress in comfortable, loose fitting clothing (ideally a t-shirt or tank top and loose fitting, lightweight shorts. Come to your visit well hydrated. Being prepared for this holistic Bowenwork therapy appointment will help your body benefit more during your first session.

Physical Therapy Prescription:
Most insurance companies require a physician's prescription and medical diagnosis for reimbursement. Please request a prescription from your healthcare provider for PT-eval and treat.
Insurance Companies Preferred Provider:
Restoration Physical Therapy is contracted with the following insurance companies: Regence Blue Shield, Premera Blue Cross, Blue Cross/Blue Shield, First Choice Health Network, Cigna, Blue Cross Blue Shield,Triwest, L&I , and accepts PIP. Restoration PT is a non-participating provider with Medicare.
